The Long Term Conditions (LTC) clinic is an innovative approach to caring for our patients with chronic health conditions such as asthma, chronic obstructive lung disease, diabetes, high blood pressure and heart disease.
Once diagnosed, you will be invited to the surgery, in the month of your birthday to have all your conditions and medications reviewed. This two-stage process begins with information gathering, self-reporting of symptoms and possible blood tests, the results of which are then reviewed by our clinicians.
You will then be contacted to say all is well, have a telephone review or asked to come in for a face-to-face appointment for further assessment. This process reduces the need for multiple visits to discuss separate issues and is an efficient use of clinician and patient time.
The LTC clinic is run by a multidisciplinary team of doctors, pharmacists, physician associates’ nurses and healthcare assistants.
